ADEQUACY
Adequate radiographic assessment of the pelvis, bilateral hips, and sacroiliac joints. Included lower lumbar spine is only partially assessed on pelvic/SI views.

Findings

Pelvis and bilateral hips

Pelvic ring is intact. No acute fracture or dislocation is identified. Femoral heads are normally located bilaterally. No femoral head collapse, crescent sign, or focal AVN-type contour deformity is visible.

There is mild bilateral hip osteoarthrosis / femoroacetabular impingement–type morphology, relatively symmetric. Both hips demonstrate mild superior-to-superolateral joint-space narrowing, JSN grade 1 bilaterally, with small acetabular rim osteophytes/rim prominence and mild acetabular subchondral sclerosis. Mild bilateral femoral head-neck offset prominence is present, compatible with possible mild cam-type morphology. No definite erosive hip arthropathy is identified.

There is mild bilateral trochanteric enthesopathic cortical irregularity. Mild ischial/inferior pubic enthesopathic irregularity is also present bilaterally. Minimal pubic symphyseal sclerosis/irregularity is present.

Sacroiliac joints

Both sacroiliac joints demonstrate chronic structural abnormality, right greater than left. The pattern includes bilateral inferior-to-mid sacroiliac subchondral sclerosis, involving the iliac side and also the sacral side, articular surface irregularity, probable small erosive/post-erosive cortical defects, and nonuniform joint-space alteration. No complete ankylosis is present.

Right sacroiliac joint: There is moderate inferior-to-mid subchondral sclerosis involving both iliac and sacral margins, with definite articular margin irregularity and probable small erosive/post-erosive defects along the inferior and mid joint margins. The joint space is nonuniform, with areas of narrowing and irregularity. No complete ankylosis is identified. Overall structural burden is compatible with chronic sacroiliitis, approximately radiographic grade 3.

Left sacroiliac joint: There is definite inferior-to-mid subchondral sclerosis, greater on the iliac side but with sacral-sided involvement also present. Articular margin irregularity and small probable erosive/post-erosive cortical defects are present, less advanced than on the right. The joint space remains preserved overall but is mildly nonuniform. No complete ankylosis is identified. Overall structural burden is compatible with chronic sacroiliitis, approximately radiographic grade 2, bordering early grade 3.

OCI-like inferior iliac-sided sclerosis is present bilaterally; however, this does not fully explain the findings because there is associated sacral-sided involvement, articular surface irregularity, probable erosive/post-erosive change, and nonuniform SI joint-space alteration.

Included lower lumbar spine

Included lower lumbar spine demonstrates mild lower lumbar facet arthropathy and mild lower lumbar degenerative spondylotic change, incompletely assessed on these pelvis/SI views. No definite syndesmophytes, flowing DISH-type ossification, or complete ankylosis are visible in the included field.

Comparison

No prior imaging is provided for comparison. This is interpreted as a single-timepoint study.

Impression

  1. Chronic bilateral sacroiliitis / post-inflammatory sacroiliac structural change, right greater than left. Findings include bilateral inferior-to-mid SI subchondral sclerosis, iliac and sacral-sided involvement, articular margin irregularity, probable small erosive/post-erosive cortical defects, and nonuniform SI joint-space alteration.
  2. Radiographic SI structural grade: right SI joint approximately grade 3; left SI joint approximately grade 2, bordering early grade 3. No complete SI ankylosis.
  3. OCI-like inferior iliac-sided sclerosis is present but is not adequate as the sole interpretation, given the associated erosive/post-erosive articular irregularity, sacral-sided sclerosis, and nonuniform SI joint-space alteration.
  4. Mild bilateral hip osteoarthrosis / FAI-type morphology, with grade 1 superior-to-superolateral joint-space narrowing, mild acetabular sclerosis/rim spurring, and possible mild bilateral cam-type femoral head-neck morphology. No erosive hip arthropathy or femoral head collapse.
  5. Mild pelvic enthesopathy, involving the trochanteric and ischial/inferior pubic entheses. In the setting of the SI findings, this is compatible with an enthesopathic / spondyloarthropathy-type structural background, although the enthesopathic changes themselves are mild and nonspecific.
  6. No definite DISH, syndesmophytes, or complete ankylosis visible on the provided radiographs.

EMR summary

Condensed for chart entry

Pelvis, bilateral hip, and SI radiographs show chronic bilateral sacroiliitis/post-inflammatory SI structural change, right greater than left, with inferior-to-mid SI sclerosis, iliac and sacral-sided involvement, articular irregularity, probable small erosive/post-erosive defects, and nonuniform SI joint-space alteration. Estimated SI grade: right approximately grade 3; left grade 2 bordering early grade 3. OCI-like inferior iliac sclerosis is present but does not fully explain the erosive/post-inflammatory pattern. Mild bilateral hip OA/FAI-type morphology and mild pelvic enthesopathy are also present. No complete SI ankylosis, definite syndesmophytes, or DISH pattern visible.

SMissing Data / Data-Needs Flags

These are research missingness flags, not clinical recommendations.

Missing data elementWhy it matters
Prior SI/pelvis radiographsNeeded to determine progression, regression, or stability
Dedicated lumbar AP/lateral radiographsNeeded for full lumbar morphology, disc spaces, alignment, and LSTV subtype
Full-spine radiographsNeeded to evaluate syndesmophytes, spinal ankylosis, DISH, and axial distribution
MRI SI jointsNeeded to evaluate active osteitis, bone marrow edema, capsulitis, synovitis, and early erosions
CT SI jointsWould better characterize erosions, sclerosis, and partial ankylosis if structural precision needed
Hip Dunn / false-profile viewsWould improve FAI morphology characterization
DEXANeeded for quantitative bone-health linkage
Lab / clinical phenotype dataNeeded only for clinical correlation; not required for image-derived structural pattern
EMG/NCS or lumbar MRINeeded for neuro-spinal concordance if radicular symptoms exist

UResearch Addendum Summary

This single-timepoint sacropelvic/hip radiographic dataset demonstrates a SI-predominant chronic inflammatory/post-inflammatory structural pattern, right greater than left, with estimated SI structural grade approximately right 3 / left 2–borderline early 3. The SI changes include bilateral sclerosis, sacral-sided involvement, articular irregularity, probable erosive/post-erosive defects, and nonuniform joint-space alteration. OCI-like inferior iliac sclerosis is present but is insufficient as the sole explanation. Additional non-SI contributors include mild bilateral hip OA / mixed FAI-type morphology, mild lower lumbar facet/spondylotic change, suspected mild lumbosacral transitional morphology, minimal pubic symphyseal degenerative/stress-related change, and mild pelvic enthesopathy. No definite DISH, syndesmophytes, complete ankylosis, hip collapse, fracture, infection pattern, or destructive lesion is visible on the provided radiographs.
